1. Medication Notice
Medications available through RXBridge are prescribed by licensed healthcare providers and sourced from licensed manufacturers and/or licensed compounding pharmacies depending on the specific treatment. Some medications may be compounded preparations — meaning they are prepared by a licensed compounding pharmacy based on your individual prescription. Compounded medications are not FDA-approved as finished products and have not undergone the same clinical testing, manufacturing review, or safety and efficacy evaluation as FDA-approved drugs. Your provider may recommend a compounded medication when a commercially manufactured alternative is unavailable, unsuitable, or when a customized formulation is clinically appropriate for your individual needs.
2. Off-Label Use
Some medications offered through this platform may be prescribed for uses that are not FDA-approved indications. My provider has determined these treatments are appropriate for my individual health needs based on my health information.
3. Provider Consent
I consent to receive medical evaluation and prescription services through RXBridge. To the extent that my care involves telehealth services, I understand that electronic communications involve potential risks including interruption of service and limitations compared to in-person care.
4. Voluntary Participation
I understand that my participation is voluntary and that I may discontinue treatment at any time with no penalty.
5. No Guarantee of Results
I understand that individual results vary and that no specific outcome has been guaranteed to me.
6. Privacy
I understand that my health information will be handled in accordance with HIPAA and RXBridge's Privacy Policy.
7. Clinical Data Limitations
I acknowledge that for some peptide therapies, long-term human clinical trial data is limited, and I have been informed of this by my provider.
